Admission Forms

To apply to ESR, complete an application form. The PDF forms will allow you to complete your application online and submit it electronically by email. You will also be able to save a copy of the PDF and/or print a copy. If you encounter technical difficulty, contact the web editor.

  1. You will need to use Acrobat Reader 7 or higher.                                  
  2. Click on the link below, either the Standard Application Form or the Occasional Student Application Form. If you just want to print the form and fill it out by hand, then click the link for the PDF print version. The form will open in a new browser window.
  3. There will be a bar at the top of the page with a button to "Highlight Fields." This may help you to see where to insert the cursor into text fields to enter your information. Required fields will highlight with red borders.
  4. Enter your information in the text fields. Some text fields allow multiple lines. There are also check boxes to click. Some allow only one choice among a group. Others receive a check to indicate affirmative.
  5. When you have completed the form, you have several options. You mmight want to save a copy to your computer for your records or print a copy. Click the button at the top of the first page to submit your application by email. Follow the online instructions to complete the email process.
  6. If you would like to submit a digital image of yourself, email the file as an attachment to esr@earlham.edu.
  7. If you are submitting a Standard Student Application, you will need to complete the Background Check Inquiry Release Form and mail (Admissions, 228 College Ave., Richmond, IN 47374) or fax (Attn. Admissions 765-983-1688.
  8. Below you will find instructions to enable you to pay the $35 application fee and the $25 background check fee online.
